By completing this registration form, I acknowledge that I have read and agree to abide by the Club's Code of Conduct and the terms of registration provided on the website here. By ticking this box, I confirm that this will serve as my electronic signature. I wish to register my child with East Herts Youth FC and understand that all membership fees must be paid and up-to-date for the player to train and play. As the individual registering the player, I will ensure that all family members and spectators adhere to the spectators' code of conduct. Additionally, I agree to pay any fines imposed by the Hertfordshire F.A. for any breaches of this code by these individuals. All kits and equipment are the property of EHYFC and must be returned to the club when the player leaves, as arranged with a club representative.
